NEW FAMILIES:
Grades 1 through 8: If you have a child in Grades 1 to 8 (Levels 1 to 8) and would like to sign up for our upcoming Faith Formation classes, there are 2 ways you can register:
1.) Electronically submit the form below (scroll down) for all students you would like registered in our program. OR
2.) Download and fill out the blank 2025-26 Registration form (scroll past electronic form) and mail to the Parish Office and/or put in the Offertory Box at Mass.  We will be in touch following receipt.  For more information, please email admin@stmattpar.org.

Grades 9 and 10+: For grades 9 and 10 or older who would like to prepare for Confirmation, please call (508) 460-9255 or email admin@stmattpar.org for more information before submitting the electronic form or sending in the paper form.  

RETURNING FAMILIES:
Families registered for FF in the previous year will receive pre-filled registration forms. If you have questions about this year's Faith Formation program, please call the Parish Office at 508-460-9255 .
FAITH FORMATION TUITION FOR 2025-26:
Per student...............................$120
Additional fee for Sacramental preparation: $55 (First Eucharist/Reconciliation (Grade 2) and Confirmation I and II (Grades 9, 10+)
